Health Information Management

With Adventist Health’s patient portal, MyAdventistHealth, you now have a direct link to access your own personal electronic health records immediately. Log in toMyAdventistHealthnow to do the following:

• View your lab results, radiology reports, and pathology reports
• Download or reprint education documents and discharge instructions
• Access your health records, such as immunizations, allergies, and medications
• View or download summaries of visits and complete health summaries.

Please note that not all medical records are viewable and/or retrievable through the patient portal. For records that are not available through the patient portal, submit a request using one of the methods below.

Our paperless request process does not require a login, but uses email verification for enhanced security. No faxing, scanning or printing will save you time.

Provider Request for Medical Records

By Paper Form

• If you prefer a paper authorization form, please download and print theAuthorization to Release Medical Information Form(English) (Spanish) and/or thePatient Directed Request Form(English) (Spanish)
• Submit a scanned copy of your completed paper authorization form by email toMedRecords@AH.org. You can also submit your completed form by fax or mail. See below for mailing addresses and fax numbers for each facility.

In-Person

• Pleasecheck the hours for your Adventist Health facilityor clinic. We encourage you to submit your request online or by paper form. Some locations do not accept walk-ins.

Requesting Other Medical Records

• Records for Deceased Patients -California,Hawaii,Oregon
• Revocation/Special Restrictions
• Copy Service and Subpoena – a separate subpoena is required for each department documents are requested from (i.e., images, specimen, or billing). For follow-up on a request, contact the department in which subpoena was served.
• Itemized Hospital Bills – Please contact your localPatient Financial Assistance. If you have a question about a physician’s office bill, contact your physician’s billing office directly.
• Radiology Imaging or Cardiology (heart catheterization) may be requested from the Diagnostic Imaging Department. Patients of Lodi Memorial please contact your Medical Records Department at the number listed below.
• Laboratory and Pathology results may be obtained from the Medical Records Department. Test samples, slides and specimens may be obtained from the Laboratory/Pathology Department.
• Risk Management – All other types of requests will be sent to the facility Risk Management department to determine the appropriate resource to respond to the request, ie: subpoena, search warrant.

Timing and Fees

Requests are typically fulfilled within 14 business days. There are no fees associated with requests sent directly to another medical provider. Fees may be associated with requests that are sent to you or to someone who is not a medical provider. If there are any fees associated with your request, you will be invoiced by our business partner, Datavant. All fees are in accordance with state and federal laws.
