Preparing for Your Appointment

Thank you for trusting Adventist Health Portland with your care. Here’s how to get ready for your visit with us at Adventist Health Portland.

What to bring with you to your appointment

  • Insurance card.
  • Debit or credit card if you have a copay. Our offices are cashless.
  • List of all medicines, vitamins and supplements that you take.
  • Any filled out paperwork we asked you to complete.
  • Any medical records and diagnostic images that we asked you for.
  • Do NOT bring weapons. Weapons are not allowed at any Adventist Health Portland location.

Before your appointment

If you are a new patient to Adventist Health Portland or your insurance has changed, please call our Patient Access team at503-261-5903and provide your insurance information.

Things to know before you go

  • 24 hour cancellation policy. As a busy health system, we try our best to see all the patients who want to receive care with us. If something comes up and you can’t make your appointment, please give usat least 24 hours’ noticeso we can offer it to a patient on our waitlist. If you miss 2 or more appointments within 12 months without 24 hours’ notice, we may contact you about no longer being a patient at our clinic.
  • Medical chaperones for sensitive exams.Adventist Health Portland offers trained chaperones for sensitive medical exams. A medical chaperone is a specially-trained staff member who makes sure you experience comfort, dignity and professionalism during your exam. We will offer you a chaperone any time there is physical or visual contact with any breast, genital or rectal areas. Although, you can ask for a chaperone for any medical exam or procedure. You can also ask for your chaperone to be of a certain gender if it makes you more comfortable. We will do our best to honor your request.
  • We are fragrance-free. Please do NOT wear cologne, perfume or other fragrance during your visit. This helps our chemically-sensitive patients and staff.
  • Language assistance services- If you need an interpreter for your appointment, we can provide one free of charge upon request. Just call your care provider’s office and ask for them to arrange language services for your visit.
