Home Care Services
We strive to provide quality care that supports the mind, body and spirit. Our home health care team partners with our patients and their physicians to develop a personalized plan of care to suit their medical needs, minimizing the need for hospital visits and stays. Our staff includes registered nurses, physical therapists, occupational therapists, speech therapists, medical social workers and home health aides.
We are a fully licensed, nonprofit organization. Service is available 24 hours a day, seven days a week in Tuolumne and Calaveras Counties. In 2014, we provided home care services for over 2,300 patients.

Provide wound and ostomy care, medication management, tube feedings, pain management, diabetic education, catheter care and other skilled services - Wound & ostomy nurse
Our agency is the only one in the Mother Lode to have a certified Wound & Ostomy nurse on staff - Physical therapists
Provide rehabilitation at home by performing home safety evaluations and developing care plans to help with gait training, strengthening and improving mobility. - Speech therapists
Provide comprehensive evaluation and treatment of speech, language, voice, swallowing and cognitive disorders - Occupational therapists
Train in the use of various assistive devices, energy conservation, personal care and meal preparation - Medical social services
Provide social services at home including emotional support and counseling for financial and/or community resources - Home health aides
Assist with personal care routines, bathing and dressing

Adventist Health Hospice - Sonora
Hospice provides compassionate quality-of-life hospice care for people who have made the decision to cease curative treatment for their terminal illness. With the support and care of our health care professionals and trained volunteers, our patients live as fully and comfortably as possible for the time they have left. Through our team-based approach, we will work with loved ones and referring physicians to develop care plans ensuring that medical, personal, spiritual and emotional needs are cared for, and that the patient's wishes are honored. Hospice has been providing exceptional care to Calaveras and Tuolumne County residents since 1982.
We understand that accepting hospice care services can be a difficult decision to make. Hospice is here to provide extra help in a time of need and to support patients and their loved ones during the final stages of life.
